Missing from hospitals, health and care settings: An analysis of Reports to Prevent Future Deaths and Safeguarding Adult Reviews

Missing People (2025)

An estimated 18% of missing adult incidents are reported from health and care settings. Tragically, a series of Prevention of Future Death Reports from Coroners, and Safeguarding Adults Reviews from Safeguarding Boards, have identified failings in the multi-agency response to these missing incidents where the person later died. This analysis considers themes and opportunities to prevent similar deaths in the future.
